Donation Tribute Card
Give the gift of charity by making a donation on behalf of your family, friends and neighbors to Community Action. The Card will be sent to them informing them of your gift.
Please click on the Donate button at the top of the page and enter in the name and address of the recipient in the “Dedicate my donation” field.
Or you may mail your donation with a request for a Tribute Card(s). Please include the name(s) and address(es) of those you wish us to notify.
Tribute Cards will be mailed within three business days of the receipt of the donation and notification.
